This form is to be completed by the person in charge of any special activity that is planned for your department, class, or group.  Please fill in all information that is applicable.  Once this request has been approved, the contact person identified on the form will be notified and the dates, nursery, space, equipment, etc., will be reserved for your activity.

  • Audio/visual assistance is available from 8:30am-3:30pm, Monday-Friday.  If you require audio/visual assistance outside these hours, please contact the church office.

CHILD CARE NEEDS

Please note:

  • 2 WEEKS NOTICE is needed for nursery notification (from the next Tuesday staff meeting).
  • There is a 3-hour maximum for the Nursery.
  • Be sure to review the NBBC Child Care policy (available from the church office).
